I would say that this would have to have occurred when the lift was being raised up under a roof overhang and was not stopped in time thus crushing him between the two and killing the unfortunate worker. Obviously, there was negligence on someone's part in this fatal accident occurring and for sure there should be an analysis of the accident and steps taken to prevent such a serious accident from occurring again.
