From the problem, we have the zeros -1, multiplicity 2. This will be (x + 1)^2

4 multiplicity 1, this will be (x - 4)

The function will be :

[tex]f(x)=(x+1)^2(x-4)[/tex]

Simplify by expanding the binomial :

[tex]f(x)=(x^2+2x+1)(x-4)[/tex]

Distribute and multiply (x - 4) to the trinomial :

[tex]\begin{gathered} f(x)=x(x^2+2x+1)-4(x^2+2x+1) \\ f(x)=(x^3+2x^2+x)-(4x^2+8x+4) \\ f(x)=x^3+2x^2+x-4x^2-8x-4 \\ f(x)=x^3-2x^2-7x-4 \end{gathered}[/tex]
